    -Address areas of concern through use of standardized assessments

    -Discuss goal setting to ensure collaboration and transparency

  • -Implement evidenced based research interventions to optimize functional skills

    -Conduct on-going needs assessment and accommodate services as needed

  • -Implement fall risk assessment to reduce risk of injury

    -Education on home modifications to improve safety awareness and reduce risk for falls and injuries.

    -Develop emergency preparedness plan of action

  • -Educate in proper handling and positioning techniques to ensure safety and optimal function

    -Review strategies to reduce caregiver burnout while continuing to provide quality care to our loved ones

    -Collaborate and communicate with interdisciplinary healthcare workers to ensure optimal care
